In January of 2015 President Obama established the Health Care Planning and Learning Action Network (HCPLAN). This entity has developed and produced multiple White Papers to assist the US health care system in transforming to population based health. Please review the Alternative Payment Model White Paper, and discuss the role of the Accountable Care Organization and how this differs from the Patient Centered Medical Home.
